Dosage & Administration Guidelines

Adult Dosage

Hypertension: Start 10 mg orally once daily (or 5 mg if on diuretic). Maintenance: 20 to 40 mg once daily. Heart Failure: Start 2.5 to 5 mg once daily; titrate upward to target 20-35 mg daily.

Pediatric Dosage

Children ≥6 years with hypertension: Start 0.07 mg/kg once daily (max initial 5 mg); titrate up to max 40 mg daily.

Elderly / Geriatric Considerations

Start with 2.5 mg to 5 mg once daily; monitor blood pressure and renal function closely.

Maximum Dose Limit

80 mg once daily (rarely exceeds 40 mg in clinical practice).

Renal Impairment:

eGFR 30-50 mL/min: Start 5 mg/day. eGFR 10-30 mL/min: Start 2.5 mg/day. eGFR <10 mL/min: 2.5 mg on dialysis days.

Hepatic Impairment:

Lisinopril is not metabolized by the liver; excreted unchanged by the kidneys. No hepatic dose adjustment required.
